Only first topic in ditamap prints

Hello,

Working in FM10… I create a ditamap and add 3 to 5 topics (mix of task, reference, concept), none are nested. After saving the ditamap, I either Save it again as PDF, print the ditamap to PS and distill, or print the ditamap using the Acrobat driver (directly to PDF). Using any of those methods only my first topic prints. When I rearrange the order in the map, still only the first topic prints (regarless of which file it is).

Any ideas?

Additional info:

DITA options: Using 1.2_Application with structapp pointing to custom template (not the FM default DITA template)

DTD is the standard 1.2 DTD, no customizations or specializations

All application mappings are the DITA 1.2 defaults
